Subject: PATH CITED Program - Acceptance Of Intergovernmental Transfer Award And
Authorization To Execute Agreement And Commit Local Match

Recommendation: Adopt A Resolution (1) Authorizing The City Administrator To Sign
And Execute An Agreement With The California Department Of Health Care Services
(DHCS) And Its Third-Party Administrator Related To The California Providing Access
And Transforming Health (PATH) Capacity And Infrastructure, Transition, Expansion And
Development (CITED) Program Intergovernmental Transfer (IGT) Award To Implement
The CalAIM-Ready Oakland Project And Deliver Enhanced Care Management And
Community Supports For High-Need Medi-Cal Population In Oakland;
(2) Accepting And Appropriating The DHCS PATH CITED IGT Award In An Amount Not
To Exceed $1,351,113.75, Which Requires Non-Federal Local Share Equal To Fifty
Percent Of The Award; And
(3) Authorizing The City To Commit The Required Non Federal Local Share Equal To
Fifty Percent Of The Total DHCS IGT Award In The Amount Of $675,556.75 In Order To
Draw Down The Corresponding Federal Match From DHCS
